A case of primary acetabular protrusion in identical twins previously not reported, and a review of the literature with special reference to roentgenographic criteria and genetic factors calls attention to bilateral crossed teardrop sign, intertuberous distance less than normal, Shenton's line intact bilaterally, and greater than normal CE angle bilaterally. The genetic transmission of protrusio acetabuli is a well documented phenomenon.
