Background: Proximal medial gastrocnemius release (PMGR) is the newest technique for addressing clinically significant gastrocnemius contracture, whose safety and efficacy has been evaluated in several modern case series. Material & Methods: This anatomic study evaluated the safety of a PMGR technique modification by testing the hypothesis that the lateral border of the semimembranosus would be easily identified under surgical conditions and would localize the approach safely away from popliteal structures. Results & Conclusions: The results of this study revealed that although the semimembranosus was easily referenced, it localized the approach with a much narrower margin for safety than suggested by previous literature.
