Temporal dispersion of contraction, arrhythmias, and dyssynchrony and role in resynchronization therapy

Abstract

Coronary artery disease is the most frequently cause of malignant arrhythmias and sudden cardiac death (SCD), but SCD can also origin from nonischemic cardiomyopathies. Left ventricular ejection fraction (LVEF) has obvious shortcomings as a predictor of patients at risk for implantable cardioverter defibrillator placement and SCD. Mechanical dispersion is a measure of heterogeneous ventricular contraction patterns and has the ability to predict ventricular arrhythmias. The temporal aspects of strain and strain-rate curves also provide insights into the electromechanical substrate of response to cardiac resynchronization therapy, left bundle branch block, myocardial scar, and other features confounding the relationship between changing left ventricular (LV) activation patterns and ventricular response. This chapter presents the use of strain parameters that record the timing of LV contraction in the management of patients with ischemic and nonischemic cardiomyopathy.
